Program Details
Our unique and individualized service will provide each client with a minimum of 3 hours of one-on-one time with their assigned case manager/coach each week in the form of coaching sessions/home check-ins and/or a mentor outing/activity. Our aim is to provide tailored support to assist our clients to gain the skills needed to create structure and balance in their life while maintaining a program of recovery. Random drug and alcohol testing will be provided every week as an added layer of accountability. Additionally, the case manager will attend weekly meetings with the Pacific Recovery team as a collaborative effort to gain feedback from supervisors and other case managers in order to best serve each individual client.
We also provide scheduled bi-weekly family calls and can be contacted anytime in between. Another key aspect of our work is in communicating with the client’s clinical providers so we can support the therapeutic goals for our clients in the work we do between sessions with their clinicians. Referrals can be provided for clinical services if these are not already in place.
Mentoring Sessions
The first session with each client will aim to determine what the client’s individual needs and goals are. We will provide resources and referrals for any additional services that the client may need or benefit from. Moving forward, we will continue to meet with the client doing one on one case management sessions. These sessions may include, but are not limited to:
Weekly assessment of the clients living conditions and offer coaching in making sure home life is stable and supports their recovery.
Healthy Relationships: Help clients navigate family and personal relationships and offer guidance on the importance of respecting and setting healthy boundaries in a tactful manner.
Employment/Vocational Support: Verify client’s current employment or offer coaching around gaining employment through resume building, assisted job search, and mock interviews.
Budgeting and Finances: Coach client on healthy budgeting skills including managing income, maintaining bill payments, and managing debt.
Recovery Program: Offer support and accountability in making sure client is actively working a recovery program including meeting attendance, sponsorship and step work.
Medication Management: Ensure client is effectively managing a medication regimen and verify that medications are on hand.
Clinical Support: Verify that client is keeping up with clinical appointments and engaging in their therapeutic or psychiatric work.
Education: Assist clients on pursuing collegiate level education including support in the application process, course enrollment, and coaching on skills such as scheduling and time management to aid in a successful academic experience.
Goal Setting: Client will set weekly goals to support their ongoing recovery and life skills. Goals are followed up on weekly to ensure accountability for follow through.
Social Engagement: Assessing to verify that our client is maintaining some sort of social engagement and is not isolating.
Physical Fitness/Exercise: Ensuring that there is some form of physical exercise or activity that is being factored into the clients weekly routine.
Drug/Alcohol Screening: Weekly drug and alcohol testing to provide accountability in maintaining abstinence.
Outing/Activity Based on Needs
An example of a weekly recovery related outing could be any of the following:
Experiential Activity: These include surfing, hiking, pickleball, golf, gym session. This promotes the benefits of engaging in healthy physical activities which have been proven to benefit mental health and recovery
Practical outing: This could include grocery shopping, DMV appointments, Doctors appointments, academic related appointments. During these outings we offer life skills coaching related to the errand.
Job Development: This includes job search, resume building, coaching around first impressions, interview skills, and the importance of follow ups.
Recovery Outing: This could include attending a recovery meeting or any other recovery related activity.
Additional Family Support
30-minute bi-weekly call with the client’s family to give updates and offer coaching around support, communication, and holding boundaries. We are available to the families at all times if situations arise. We firmly believe that an open line of communication is key for success.
Regular contact with the client’s clinical team (therapist, psychiatrist, etc.) to discuss progress, medication management, and how we can best serve their therapeutic process.
Immediate support in crisis management to provide support and resources in the event of Drug / Alcohol Relapse or Mental Health Emergency.
